The top of the sky: the meridian where this planet stood highest at the minute you were born. It is the angle of the public record, and it governs work, standing, and the thing a person ends up known for, which is not always the thing they set out to do.
A Midheaven line is a meridian. It runs pole to pole at a single longitude, which is why it draws straight while the horizon lines curve.
Pluto at the Midheaven makes the public sphere a place of consequence. Influence here is real and rarely comfortable: the work tends to involve other people's crises, other people's money or other people's secrets, and it attracts scrutiny in proportion. It is formidable ground for anyone whose vocation is transformation and punishing ground for anyone who simply wanted a good job.

Power becomes the subject. The work here involves control, crisis, hidden structures and the tearing down of what has gone rotten, and you will not be a bystander in any of it. Careers under this line transform rather than progress.
You will make enemies, and some of them will be patient. Politics turn personal, ambition can go ruthless without your noticing the moment it did, and what is hidden in this place has a way of surfacing in public.
Ask this line about a vocation rather than about a job: work that deals with crisis, money, power, or the things people do not say out loud. It suits that. It is punishing to weigh for a decision about a quieter career, and the scrutiny it attracts is not optional once the influence is real.
The line for a complete professional rebirth, for taking genuine command, for the work of exposing what others protect. It will demand that you survive being opposed and cost you the comfort of being liked, and it will yield an authority that cannot be inherited or bought.
Pluto is one of the five bodies the tradition calls difficult, and all twenty of those entries carry a passage like this one. A demanding line is not a verdict.

Nowhere in the gazetteer sits within 80 km of this line: no city of 100,000 people or more, and no national capital. That is a real answer rather than a missing one. This line spends its length over water, ice or ground nobody has built a city on.

The distances are measured. Each is the shortest great-circle distance from a real settlement to the exact curve of the line, computed against a committed public-domain gazetteer by the same engine that measures a paying customer's own.
The interpretation is a different kind of thing and we would rather say so than blur it. It is craft, written by hand in the tradition locational astrologers have refined since the nineteen seventies, and it describes what a place tends to support and to demand. It is not a prediction, and no line on any map decides what happens to a person.
