What a Midheaven line is.

The top of the sky: the meridian where this planet stood highest at the minute you were born. It is the angle of the public record, and it governs work, standing, and the thing a person ends up known for, which is not always the thing they set out to do.

A Midheaven line is a meridian. It runs pole to pole at a single longitude, which is why it draws straight while the horizon lines curve.

The Midheaven is the highest point the sky reaches over a place, and the Sun there is the plainest statement astrocartography makes. Nothing about the meaning is subtle, which is its own kind of usefulness: this is the line to read when the question is about standing rather than about feeling, and the one most likely to sit badly with someone who works best out of sight.
